Preparing for Appointment
The following are suggestions to help make your visit with Dr. Johans as effective and efficient as possible. Dr. Johans goal is to make excellent use of your time to avoid future visits due to incomplete or inaccurate information on the initial visit.
Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire. Please have all forms filled out prior to arriving to office. These forms are located in the patient forms tab on website.
When you come for your appointments, please remember to bring the following
- Driver's license or State ID
- Insurance information
- Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits
- Medications list (Please write down medications on Intake Form exactly as is written on prescription or obtain pharmacy records)
If you have had surgery elsewhere, please bring a copy of your operation report. Very often during the course of your evaluation or treatment for an neurosurgical condition we will require x-rays to determine the cause of a problem or to evaluate your progress.
It is essential that Dr. Johans has your surgical history as accurately as you are able to convey. Operative notes from past surgeons would be very helpful if the surgery is related to your present issue.
What to expect at Initial Appointment
- Interaction with either Physician's Asstiant or Physician. Be aware that the Physician is always overseeing cases.
- Be prepared to answer questions about your condition.
- When did pain, numbness, or weakness start?
- Where does it radiate to? Exactly..
- What helps and hurts ?
Please write down all the questions you have prior to office visit.
